The purpose of the Linguistic Services Program is to assist you (the client) in accessing and understanding your medical care and other related services. Services are available if you live in or have appointments in Albany, Rensselaer, Schenectady, Fulton and Montgomery Counties only. Someone will meet you in person at your appointment if confirmed, but interpretation may on occasion be approved for telephone contact. The program funds must be the “payer of last resort.” Therefore, all other interpretation/translation resources must be used prior to using this program’s. The Linguistic Services Program is not an emergency; appointments must be scheduled at least one week in advance. This Client Information Packet contains: • Client Agreement Form In addition to the these documents, an Authorization for Release of Confidential HIV Related Information (HIPPA HIV Release) will be completed which identifies Catholic Charities AIDS Services and its linguistic vendors, the Provider Agency and medical provider to enable communication regarding this service delivery. The Provider Agency will also be submitting information on an AIRS Data form in order to complete service registration. Catholic Charities AIDS Services HIPAA Privacy Practices are distributed to provide information on the safe keeping of client data and information.
