Supportive Case Management is offered to assist those infected with HIV/AIDS to access and coordinate needed medical, financial, psychosocial, mental health, substance abuse treatment and other needed services.   

Referrals by providers or self-referrals may be made to our Albany office for residents of Albany, Rensselaer, Columbia and Greene Counties by calling 449-3581.  Residents of Schenectady, Fulton and Montgomery Counties may call the Schenectady Site at 382-2969 for assistance. Services are free and potential referrals do not need Medicaid or other insurance coverage to enroll.
